Summary
The past decade has seen many changes in the health care delivery environment. While many have affected financial access to care, and thus, health outcomes, others are likely to affect the health of diabetics and hypertensives by changing the organizational climate in which care occurs. The poor continue to be affected by these changes. Because they begin with a higher burden of illness and have worse outcomes, they deserve special attention.
